Instructions

Please fill out the form in its entirety.  Every box must be completed.  If the form requests medications or medical conditions and there are none, simply put "none" in the box.  If the form requests a number and there is no number, simple use zeros.  When you get to the end of the form and select "next" to complete the registration process and the next page doesn't appear, then you will need to review the form and complete missing information. You will see a very small red dot by the missing information.  

A form will need to be completed for each participant.  The Release will be in effect January 1, 2023 - December 31, 2023.

Please note that this Release will be in full force and effect for all Springcreek events. However, some events may require additional participation releases by the hosting entity.
